Instructions for Completing the Carter Leasing Service Work Order Form

Follow these steps to ensure your service request is processed quickly and accurately:


1. Date & Time
The date and time when you fill out the form are automatically recorded.

2. Customer Information / Contact Info
Include the name of your company or organization. Please provide the contact person’s name, phone number, and email address regarding this request.

3. Site Location
Enter the full address where the service or unit is located. Include any details that will help locate the building on-site.

4. Modular Building Details
List building type and related serial or identification number(s) of the modular building(s) that need service. You can usually find this information near the main door or on earlier lease paperwork.

5. Description of Work Needed
Briefly describe the issue or service request. Examples:

  • “HVAC not functioning in Unit A.”
  • “Requesting relocation of Unit 2 to a new location on site.”
  • “Pickup of temporary office unit following project completion.”

Write the type of work needed:

  • Installation
  • Maintenance
  • Repair
  • Relocation
  • Removal
  • Other (specify)

    Special Notes / Site Access Instructions
    Note anything the service team should know, such as:

    • Gate codes or restricted access details
    • On-site contact person
    • Safety requirements or hours of operation
